It is refreshing to hear the heartfelt perspectives of a Physician about ACOs. Those who practice medicine, those who develop the guidelines regulating the practice of medicine, and those of us that provide the supporting technology - need to engage in more dialogue around clinical guidelines for caring for chronically ill patients. Hopefully, clinical guidelines developed by the medical societies will be used as the basis for regulatory and financial guidelines. This would go a long way towards enabling more cost effective care, while avoiding the creation of yet another set of perverse incentives which drive the healthcare system today.

Matt, thanks for your comment. Spot on with the Microsoft remark - in fact, came across this (http://www.networkworld.com/news/2011/121511-ie-update-254097.html?hpg1=bn) today to drive that point home.

I can understand why it's easy to be skeptical. However, there are examples all around us to show that things are starting to change:
- OptumInsight's CareTracker EHR is 100% cloud based, and so is our competitor PracticeFusion
- CareCloud, a startup that just raised $20M, offers practice management in the cloud
- OptumInsight's Axolotl HIE is also cloud based

Additionally, small and large vendors are starting to offer open APIs for third party application integration with their clouds - I'll cite Axolotl's Open Access platform, PracticeFusion's API etc. as examples.

So yes, there's a long way to go but I think the momentum is building.

Very valid points here, Mr. Schroff. I've worked in this industry for a few years in various roles for a vendor and a hospital and can say most (including yours) are far too slow to adopt even versions of MS software running in the background. Nice to hear about the cloud, but I'm not betting we'll see that happen any time soon in healthcare based on my experience.
